We WELCOME all Patients with or without insurance. For accepted insurance carriers see the Insurance [link to PDF] list and Insurance Information Authorization form or call the center (703) 532-2500. It will be greatly appreciated if you provide your insurance information to the scheduler when you make your appointment so we can verify your benefits prior to your appointment.
Payment or Insurance co-pay is due at the time your services are provided and may be paid by cash, check, MasterCard, Visa, American Express, money orders, traveler's checks, or cashier's checks. Personal checks ARE accepted through Tele–Check with an identification card. Itemized statements can be provided if you want to file your own health account claim.
