Virginia Requires Pre-Information:
The Virginia State Legislature requires that five points of information be shared with each patient who makes an appointment for a pregnancy termination.

The PDF files in the FORMS list on the Patient Information Page provides that information prepared by our clinician which can be reviewed with one of our Health Educators or Nurse.

More detailed information regarding the procedure will be discussed during your registration on the day of your appointment.

Certification of Pre-Information is required to be kept with your chart. After reading the following pre-information, you can print out the Certification of Pre-Information form and bring it with you to your scheduled appointment or use the form we will supply to you.

Virginia Parental Consent :
An unemancipated minor who has not yet reached her 18th birthday is required to have the consent of a parent to end a pregnancy. The explanation of the Virginia Law and the necessary forms are in the FORMS list on the Patient Information Page. The parent must have proof of identity and have signature notarized. FCHC has notaries on site and can provide the notary service free of charge.
